IT'S NOW SAFE TO ENTER THE
NCMA / ICPI SAFETY AWARDS PROGRAM!
The Safety Awards Program recognizes safety-conscious Producer and Associate Members of the National Concrete Masonry Association (NCMA) and the Interlocking Concrete Pavement Institute (ICPI), as evidenced by exemplary safety records. Since the program's inception, participation has grown steadily--from just over 40 entrants four years ago to more than 100 last year!
The 2009 Safety Awards will be presented during the 2010 International Concrete Exposition (Icon Expo). Award recipients will also be recognized in Concrete Masonry Designs and in other trade publications.
PROGRAM REQUIREMENTS
The NCMA / ICPI Safety Awards Program is open to all production or manufacturing facilities of Producer or Associate Members of NCMA or ICPI in good standing during the reporting period. Each facility enters individually; there is a discount for multiple-facility entries from the same company.
The awards are for the reporting period of 1 January 2009 through 31 December 2009.
To enter, complete and submit a copy of the facility's OSHA 300 log or equivalent Canadian injury report, covering the entire reporting period, with the Safety Awards Program Entry Form and entry fee, by 21 January 2010.
The Milestone Award has been established to recognize 5, 8, and 10 consecutive years of zero-recordable-injury Safety Awards Program entries; make sure you preserve your future eligibility for this prestigious award by entering all of your zero-injury facilities this year!
Entry Fee
There is a base fee of $295 (US Dollars) for the first facility entry from each company, with a discounted fee of $150 for each additional facility entry from the same company.
The base fee includes one ticket to the awards ceremony; additional tickets may be purchased for $85 per ticket.
Companies entering multiple winning facilities will receive a company plaque; each winning facility will also receive its own plaque.

The entry form is a fill-in PDF and can be completed on your computer; if payment is by credit card, the completed entry form and supporting documentation can be submitted by email to drozsa@ncma.org. Alternatively, you may print it out and FAX it to 703-713-1910 or mail it to the address on the bottom of the form.
